About TRAC
TRAC (Transformation Resource for Accountable Care) is a collaborative initiative created by the NCMS Foundation to help those who deliver, experience, and support care navigate the shift toward value-based, accountable care.
Designed as a centralized hub for education, tools, and collaboration, TRAC connects clinicians, community partners, payers, and consumers with practical resources and shared strategies that make health care transformation more achievable.
Developed in response to the growing complexity of care delivery and value-based models, TRAC helps turn complex systems into actionable guidance—supporting more coordinated, equitable, and person-centered care across North Carolina.
What TRAC Does
